Sam Adams Utopia at ~25%, but it only had a limited run. You can still find it but you're paying collectors prices for it. For the highest alcohol in a continually produced beer, you'll have to go with a Dogfish Head 120(ale) or World Wide Stout(ale), depending on vintage. both of those beer are in excess of 19%, again depending on vintage. Samichlaus as mentioned before has the highest alcohol for a lager (vs. an ale (which are both beers, it's a matter of yeast used)).

Some of those Dogfish Head beers are way up there, like around 20%. They are really thick and almost taste like liquor, but are cosidered beer.
