What are you smoking. Only graves that are higher than the paths are tombs an mausoleams. Other than that only thing that protudes above the grave is the headstone and grave edgings

Because people walk along the paths eroding them, also in some older graveyards there has been a certain amount of re-use causing more material to be deposited in the burial areas.
Also churches tend to be built on top of hills, therefore the ground surrounding the church will be higher than the road running past it.

And what of Native american burial mounds?
Actually I think the most likely answer is that normally the church is next to the grave yard, and although having the graves so much higher may not have had much of a prupose, having the church on a higher portion of land was significant. sake of worship. the idea of what you worship is placed above you, higher ground.
and so having the graveyard on higher ground just became a side affect.

Burying bodies increases the volume of material which will tend to cause some of the rise. Otherwise it will be for drainage purposes

Where I'm from one of the walls broke and the graves spilled out into the street. It happened before I was born, but I heard about it.

So they can bury people in them without hitting solid floor as they are filled in plus as they put More people in they need space for excess soil and then be Abel to put it back as the body decomposes

Not too sure really, but I know that my Granny's is real big, but there are 6 people in there. So maybe it's more popular to have family graves in UK?
