S&M generally refers to sadism and masochism. This is the practice of where one person (the sadist) will inflict pain upon the other (the masochist). It's a common alternative sexual activity, and it can even be non-sexual.

First, to clarify, pain is not the same thing as harm. When pain is inflicted in certain areas of the body (the buttocks being a common location), the nerve sensors of the masochist tells the brain to produce endorphins in order to counteract the pain sensation. The production of endorphins can actually stimulate the pleasure centres of the brain, which in turn helps generate more endorphins. If the infliction of pain continues long enough, it is possible for the masochism to be overwhelmed by the endorphins to the point of feeling "high". For the sadist, it can produce similar sensations as the feelings of control can stimulate the pleasure centres of their brains as well.

S&M is not for everyone. Indeed, some people will find it repugnant. However, for the hypothetical married couple, it is their own business, and the stimulation of pleasure centres can lead to a greater level of intimacy.
