You don't mention her age or how long you have been married!!
Some women would love victoria's secret type gifts, other are more Tiffany's, those of us a bit...more....mature prefer somewhat practical--like Williams Sonoma. If she likes wine, how about a fabulous bottle of wine, with some caviar (real stuff, not grocery store).
What catalogs does she tend to look through when they come in the mail? That might give you hints as to what sort of things she likes.
Theater tickets (to a play or musical) would work if you live near some.

IF you have kids, arrange a baby-sitter right off. make an appointment somewhere with a massage therepist for 1/2 hour, and let her get a massage. Next, take her to a nail saloon to give her a manicure, and or pedicure. Top it off with a nice dinner at a restaurant (and if you can afford an extra gift, or a sappy card- give it to her at this time). She will never forget this b-day!!!

love and happiness. trust and careness . proved to her that she is for you everything. Take her to her favourite place or restaurant and orgarnise the surprise Birthday party and invite all her friends.decorate the place with red roses and ballons.And just give her bunch of red roses . Thats enough to make her happy, sometimes this is much better than a diamond ring.

You could get her a pink or ice blue diamond necklace with ring to match or perfume, if you know what she really likes just take it from there.

think of sumthing that shes really wanted....if thres nuthin special gift her ,her fav brand of perfume,lingerie,her fav dress she eyed at the mall,a candle lit dinner,the best cld be if u could take her out for a holiday! happy bday to her..and hv fun!!!1
