it's not from his mouth. from the nose? antibiotics didn't clear it up, and it comes and goes frequently without rhyme or reason.

This is caused by an accumulation of infected mucous in your sinuses. My husband had the same thing. Sometimes the odor was so bad, I couldn't stand next to him. Antibiotics didn't help, because by the time it gets this bad, it needs to be suctioned out. Go see a Ear, Nose and Throat Specialist.

Sinus discharge can often cause this.
If it is infection, the RIGHT kind of antibiotic should be used.

Drink lots and lots of water and keep nose clean. Use Bactricin on it three times a day. (Wipe a little on the edges with a q-tip)

Sometimes you can get a sinus infection that is resistent to treatment.
You could also have a nasal polyp that could come and go, and when it becomes stagnent it could stink.
In a little kid, you are always thinking foreign bodies being shoved up the nose, especially tic tacs.
Have an ENT see your friend.

Possibly a mold or other fungal infection that is provoked by the natural cyclic changes in body chemistry. Or it could be debris lodged in there.

my kids went thru that phase. one stuffed a piece of sponge up their nose - smelled bad.

another one jammed a piece of BBQ pork in their booger hole - it smelled like something died... nasty.

but those didn't come and go - until the doctor pulled them out.
