Not all rebate forms go there, of course.

What you really want to know is why there are specific places in the US - Minneapolis and it suburbs, Omaha, Dallas, and Denver or Colorado Springs - that seem to be the centers for direct mail activities.

The reasons are threefold - postal rates, printing capacity, and pre-existing business structures to serve those markets competitively.

Most of those cities are locations from which postal rates are very attractive, depending on the spread of the markets served.

Minneapolis, Dallas and Denver happen to have exceptionally good and competitive large printing plants. MANY magazines are printed in those cities, for example, combining attractive postal rates with really good printing prices.

Finally, some cities, thanks to their early positioning in both postal zones and telephone switching centers (or optical fiber connections) developed very strong and competitively-priced service centers. Omaha is one such.
