How many lives have been lost building these tunnels, both workers and commuters?

Besides the woman who was recently killed, how many more people have been killed in the tunnels...Both when being built or just regular traffic?

The woman in the car that was crushed by the concrete tiles is the only fatality of the big dig. There have been other construction fatalities, but not having to do with the big dig.

the recent failure of the tunnel nuts killed a women in her car.
