Is there a list anywhere showing which countries include fluoride in their water supply?

There is a map at the site listed below (PDF file) which shows the countries with "endemic fluorosis due to excess fluoride in drinking water."

Water fluoridization is done at the county or city level since those are the governments that control water supplies.

I didn't find one, but fluoride isn't all it's cracked up to be. Have a look at that website.
