The bible as we know it was first adopted by the Nicene Council in 325 CE from an assortment of different religious scrolls.
The Catholic church formally adopted the "bible" in 397 CE at the Council of Carthage.
That book was edited by King James in 1611 to demonstrate his loyalty to the protestant movement. He was born a Catholic.
No one know where the stories the Catholic church adopted came from or who wrote them.

God inspired men all the way from Moses to the Apostle Paul and John to write the Bible. After the Bible was written, some people tried to destroy and take it out of context. However in 1611, King James of England took the original manuscripts and translated into English...from then on the Word has pretty much been free to everyone. Except of course with the church of England later on when they didn't want everyone to have their own Bibles because they knew that what they were teaching wasn't in the Bible. That is why the pilgrims left and came to America- they wanted religious freedom to worship God freely and how the Bible said.

Depends on your beliefs I suppose, but there is evidence to suggest that a historical Moses did exist. It is likely that the name 'Moses' is only a Hebrew version of an Egyptian name; probably Ra moses (son of Ra) . It was this figure who first collected the tribal history of the Israelites into written form. More than likely he was acquainted with the mythologies of other local tribal groups and amalgamated those into his writing. This explains why the Flood Myth appears in many traditions.

That is a question that thelogians have been debating forever. Many, many more books have been written and not included in the bible that you know today.
