Don't know about Iran. But North Korea is another issue. If they launched a ICBM at us. We would respond. If I am not mistaken after 9/11 . We took out the 1st strike. Which means we would have to be nuked 1st to nuke them. Now we could nuke them 1st.

Not a chance. With North Korea we have China to contend with as well as Russia, the same can be said with Iran. We are not as powerful as we once were with our irons in the fire in Afghanistan and Iraq. We are severely limited in our options.

If things reach a point where we feel as if they have become a direct threat to us, then I believe it could well happen. Would probably be tactical nukes designed to take out their programs.
