Does it really matter? It is that person's business. Don't judge, don't assume. It shouldn't bother you at all as it has no impact on you.

It's definitely worse to think a fat person is pregnant. Think about those singletons like 'bridget jones'. It would be adding insult to injury. That's also a general sweeping statement thrown to fat girls. A pregnant person is fat since the foetus obtains nutrition from the mother. However, not all women put on massive weight during pregnancy. Especially those who are anaemic.

I think it's generally bad to assume things about people. Better is to find out the truth rather than assume. Assumptions can be insulting, not to mention how annoying and tirening it can be to get them all the time.
But to answer your question, I think it's more insulting when a fat person gets asked if she's pregnant than when a pregnant person is thought to be fat. I'm sure both can be insulting though.

Etiquette isn't about your thoughts, it's about your behavior. If you didn't say anything (body language counts as language, btw), then the concept of "rudeness" doesn't apply.
But suppose the lady had been wearing one of those cute maternity tee-shirts with an announcement printed across the tummy "Baby Girl due in October" -- and you had been standing there with a smoke in your mouth and the pack in your hand. Would you have taken it upon yourself to try to prevent or curtail this lady's smoking?

I've learned to keep my mouth shut and never asked the woman when the baby is due, because most of the time, it turns out that she was just overweight. I don't want to insult people like that. So I say nothing and wait until they mention it. Weight is a sensitive issue for people.

It's worse to think a fat person is pregnant because they're fat for no reason. Atleast a pregnant person has a reason to be bigger.

It depends: which would you find more disturbing, considering the individual in front of you?
Oh, and, P.S. None of our business if a pregnant woman smokes, has a beer, or wants to take sky-diving lessons, if she is a stranger to us. She is accountable for her own actions.

It doesn't matter what you think, but if you ask a fat person if they are pregnant that is very insulting.

First of all, quit smoking. it's bad for your health. Second of all, whether I'm fat or pregnant is none of your business. Third of all, your smoking can affect my health. My being fat has no bearing on your health. (Jumping off soap box now)
