Mr. Noodle and his brother, Mr. Noodle are just imaginary friends in Elmo's World... Many kids have imaginary friends and that's their take of that on Sesame Street... Sesame Street teaches diversity. So, I think that's why they chose people that aren't normally thought of as "tv attractive" for this segment so that children learn about the real world without prejudices...
