With most social phobias, what you really need to do is practice. Force yourself a little at a time.
The guy at Subway is a guy like you, he has mostly the same hopes and fears that you do and is probably a decent guy, why be afraid of him?
Get involved with some group, a hobby club or some sort of social club. It's easier to open up around people that share common interests, and it will help build your confidence.

You are likely to have low self esteem, in my opinion. I think you should practice shouting to yourself in private, to get used to using your voice in this manner. Then start trying to raise your voice at Subway gradually. You will find that it is easier than you think. And so what if people turn and look at you? You are just trying to place an order, for goodness sake. As long as you are polite, you will receive the same kindness in return, most of the time.

That sounds like social anxiety. You can talk to your doctor about this, it is a treatable condition. To work on it yourself. Practice yelling in your back yard with a friend. Try to have a good time and laugh at it. If you can do that, you can actually practice it in public. Have your friend move away from you so that you have to call out to him. Do so and keep in mind that others can hear you. It's okay for that, so you should be okay other places too. Hang in there, you can get through this and you can get it treated.
