Usually blood pressure, height, weight, check for hernia, (embarrassing for most teens the first time) and update shots if necessary. A really good will doctor ask for a urine sample and test for anything out of the ordinary. Drug tests are done by court or parental request. HIV and sexually transmitted disease tests will be done by patient or parental request.
