Yes. You have a right to your medical records. The way I do it is the day I go in for a doctor's visit, I tell the doctor up front that I like to have a copy of my medical records and test results for my home records and own personal use. They have always complied with no problems and at no charge. Some, however, ask you to sign a release form and may charge you about $4, but you can still get them. It is best to ask for them the day you go in.
They are also most helpful when moving or seeking another physician as you won't have to write the office and ask for them and you'll have them up front and the new office can then make COPIES of them and this also helps speed things up.

Yes you should be able to; the information that the hospital, doctors surgery have on you is about you therefore you have a right to see that information and request a copy of that information. This is according to the Data Protection Act within the UK. I don't know where your from so, but how data is used and protected is similar to other countries so. But yeah you should be able to. If you ask and they say no then you should ask a lawyer what you can do about it.

Yes, in Canada legally they have to provide you with a copy upon request as per federal privacy and confidentiality legislation, but can (and probably will) charge you a fee for providing it, to cover copying, paper, administrative costs including the time it takes to copy each page.

Ask your doctor. There may be a small fee for the copy machine, but they are legally yours. If he/she refuses, report them to the AMA and the local authorities. Lawyers LOVE to pick on doctors.
