I would reccomend a general anesthesia for your extractions. Just for the simple fact that your mouth will need to opened wide enough to extract the teeth. A local will not relieve TMJ pain. Talk to your dentist about fabricating a night guard for you. Sometimes this can relieve some of the tension put on to the TMJ. There are many different causes for disfunction of your TMJ. Grinding and clenching seems to come out on top from my personal experience. Having one of these appliances made is a simple procedure. At your first visit impressions will be taken of your upper and lower teeth. The following visit your appliance will be tryed in and you should walk out of the office with it. I hope this will help. Usually the last resort would be to see an oral surgeon. Good luck!

I have TMJ also, but it's not too severe, and I'm having my wisdom teeth out this winter... I've always had problems with getting teether out but I never got a bite plate... I'm not sure what it will do but I don't see why it would aggravate it because your teeth and jaw should both be adjusted by now... I hope everything goes okay!!

I actually heard that getting wisdom teeth out helps with TMJ problems.

It can, but that depends on the current health of your TM Joints. Find a dentist that uses objective diagnostics to evaluate the health of the TMJ. If your dentist or oral surgeon blows off your concerns, or does not use diagnostic tools (more than just a stethoscope or panoramic xray) to diagnose the health of your tmj they're simply guessing at your health.

Be sure the dentist or surgeon knows about your TMJ history. They will try to take it easy on you without having you open TOO much. Hopefully.

it will probably aggravate it...i think i have tmj also, but when i asked my doc. and my dentist all they said was it is possible then dropped it. any kind of work done on your mouth has the ability to aggravate it..just let the oral surgeon know ahead of time.

Just let your dentist know that you have TMJ and he will take the necessary precautions to keep from aggrevating it.
I feel for you, TMJ is SOOOOOO PAINFUL. :'(
