No it is not. Maintenance and repairs usually do not require a building permit. Just to be on the safe side, you may want to call your local building inspectors office for their advice, but I am all but certain that you won't need a permit.

I would say no to the toilet, but as to the floor, it depends how much you're replacing. If it's just pulling up old tile and laying new tile, I really doubt it. Both are less than dangerous jobs.
Just don't forget to plug the toilet pipe with a large damp rag to prevent methane fumes from entering your home while there's no toilet seated on the pipe with water in the bowl.

To be sure, pop over to your town's city hall and ask about building permits. They will be able to answer your questions.

It depends on where you live, but I don't think you do because you're not doing any real construction, electrical, or plumbing work.

Unless by floor you mean the actual entire floor rather than just new carpet or tile.

Probably, but your doing this indoors who is ever going to find out. Buy something nice with the money you save on your permit.

No. Only when you change the outside structure as in adding an addtional room or building where people will be living, then it has to pass code to be considered safe.
