Definitely friends name.... and guest... that way if friend doesn't want boyfriend to come, she can come by herself, but, if she wants to invite boyfriend, then she can. not obligated either way.
I'd do it with the invites of friends that have boyfriends/girlfriends that you don't know their names, otherwise, if you know their names, add their names to the invitation of the friend you are inviting

I think that if it's a close friend you might want to put his name as well - but the best thing is to check with her beforehand to see if he's really coming. Right before we sent out the invitations to our wedding, my husband and I called our single friends and asked if there was someone they were definitely bringing - those people we printed the names of their boy/girlfriends. The people who didn't know for sure who they were bringing, we just wrote "and guest."

If you know the sig. other's name, then you write that in . You only resort to 'and guest' if there is no sig. other and you want them to be able to bring someone.

In this case, I would address the envelopes like this:

(Outer Envelope)
Ms. Kathryn Smith

(Inner Envelope)
Ms. Smith and Mr. (Boyfriend's last name)

If you are doing an informal style, I would do the following:

(Outer)
Ms. Kathryn Smith

(Inner)
Kathryn & Jonathan

or for really informal inner
Katie & Johnny

Married Couples, whether or not you know the spouse, look like this:

(O)Mr. & Mrs. Jonathan Smith
(I) Mr. & Mrs. Smith

Unmarried Couples living together:
(O) Ms. Susan Adams
Mr. Jonathan Kent
(I) Ms. Adams & Mr. Kent
(The names are listed alphabetically)
For a married couple with different last names, you wouuld include an "and" between the names on the outer envelope.

Again, everyone who is in an LTR should have their sig other's name mentioned.

You should write your friend name and then write guest on the invite.
