Because kids are mean and they pick on the different kid as if to say "you are different - I am not different - see, I pick on the different kid - I am the same as the majority".
You see, school is the only environment in life where conformity is prized over all else (because a school churning out that many kids has little time for individuality) and you are confined with other kids who know far too many details of your life.
In that position, I would consider making a stand over it with those boys - since you will only have to do it once (they are always the same class, right?). I would be tempted to say in a very bored voice "Oh you are observant - yes, I am over weight. How perceptive! Moving on..." - like Stewie Griffin - and then ignore them.

I would strongly encourage him to lose weight just for health reasons, obviously. But he needs to work on his self-esteem. He is an easy target, so the kids will always target him. If he builds up his self-esteem and confidence, then he can either let it roll off him, or he can say things back. Like I would tell him to make fun of their johnson's, because guys hate that and seem to be more self-conscious about that than anything. He can also use humor, you know how most fat people are funny. He has to do something or it's gonna be Columbine.

His self esteem is low, and the bullies of the school know this and KNOW they can belittle him without fear of retaliation. Bullies have their own brand of character defect. For them to feel good, they must make others feel bad.
Having his parents butt in, or even worse, the school administration, he will still be at the mercy of the bullies.
First and foremost, he needs to have his diet and exercise programs ok'd by a doctor, to make sure he does not have a serious problem.
While the first needs are addressed, your cousin needs to be counseled by a therapist who specializes in children. Your cousin is in for a life of the same if he does not get help now.

Kids are mean.
Hormonal boys are very bad.
I remember several boys would never take a shower after gym because of all the mean kids. It sucked for them.
I wasn't fat but I got picked on because I wouldn't fight. I finally snapped one day & fought back. The bullies found somebody else to pick on.
I do not recommend anyone fighting but kids need to stand up for themselves & their friends.

The fat kid is the easy target. A pack of guys will always go for an easy, soft target to make fun of....it's human nature. The fat kid is shy and has no Friends to back him up...he is all alone and sticks out. They also are sensitive...the slightest comment will get a reaction out of them.

Anyhow, he should file a harassment complaint within the school. His parents should get involved. The kid does not deserve to be teased.
If the school does nothing, then take it a step further.

They laugh at him because they are insecure with themselves. They also do it because it gets a reaction out of him.
Tell him to walk tall and proud like he has the best built body there. When they stop getting a reaction from him, they will let up.

children are cruel. there's no way around it. people who are overweight always get picked on. the only thing you can do is help him start an exercise program and try to help him boost his self esteem.
