Asking a girl out depends on the situation. If you already know her and have set up a dialog with her (talk with her sometimes), then during one of your conversations just say something like, "I was thinking of going to (ball game, movies, the park) next Friday evening. Would you like to go with me?"
If you have just met her, wait for a moment when she looks at you with hungry eyes and then do the same (unless she asks you first).

go up to her n say "hi my name is(.....) and i like u, would u like to go out with me?"...if she like u she'll say yes n if not u get over it n keep on moving.
