Better when done slowly, as that requires more actual muscle use instead of momentum. Try it out -- it's a lot harder to go slower, right? That means your muscles are working harder!

Yes, done slowly. But unlike everyone else, not ALL exercises are best when done slowly. If you want to gain more jumping ability one of the best exercises is squats and the best way to do them is power lifting the weight up as fast as possible.
For Push-up and 98% of all other exercises slow and controlled is best!

Yes and no. When you exert force, you should exert it with an exceptional amount of force. Of course you don't want to exxagerate this by flying up as fast as you can. When coming back down, that's where you should slow it down a bit.
Keep in mind to build a muscular chest, you need to lift weight. Your body will eventually get used to the number of push-ups you do, and you'll need to challenge your muscles by increasing the weight.

Alternate from fast to slow as a sort of "break" do 10 fast, then 5 slow and do it till you can't do anymore ... thats how muscle tissue is built. But make sure you don't overexcercize...you need at least two days a week off to let he muscles heal and thats when you notice em bigger.

Do them slowly, and to add variety, lift one foot off the ground during the push-up. It will work still more muscles.

Ten pushups at half speed equal five pushups at regular (twice the half) speed.
The formula for "work" done I believe is joulles (force) over a certain amount of time ... but I don't know the math for sure, sorry.
