Diapers are always good. Or you could go for the gift card option. This was they can go buy anything they want they don't already have. What i usually do is buy random things for the baby example: diapers, wash cloths things of this sort and blase them in a basket. Also include something for the new parents such as a gift card for a dinner or something of this nature

Go to your local jewelry store and get a silver rattle or brush set. They are fairly inexpensive ($20-$55) and can be engraved and personalized. These are also something that most parents won't receive. I mean, many get the silver cup and spoon, but rattles, teething rings, and brush sets are just pooh-poohed. I sent my nephew a silver teether engraved with his name and b-day and my bro and sis still rave about it!
Some parents really love ID bracelets or Baby Bangles if you want to invest a bit more in the gift (not sure of your price range!). They are really cute and something the kid will always have as a reminder of their baby days.

I say get something for the parents. If they have lots of baby stuff, they would certainly appreciate it, but it would be nice if it were something for just the two of them. Wine tour or a gift certificate for a nice dinner or a big bottle of booze!

Depending on your budget.....A card of congratulations, a basket of goodies that they can use (food, gift certificates to target, restaurants, or Babies R Us;) a photo album for photos of the baby, a gift certificate for photo processing, diapers (larger sizes too), anything that is delivered to them so that they don't have to go anywhere (magazine subscription for a parenting magazine).
You could also offer to have their car washed, their house cleaned, go grocery shopping for them.... anything to save them time so that they can concentrate on caring for and loving their baby.
Just don't get them baby clothes -- guaranteed they already have too many clothing items.

I always throw together a basket with baby nail clippers, diapers, wipes, wash, lotion, powder and oil. They will love it, you always get tons of clothes and toys, but never get the stuff you can really use and need right away.
