I want to make sure his b'day isn't forgotten amidst all the wedding celebrations - if I could surprise him by standing up in front of all and say to him " I've done/got this for you ......................." Haven't a clue what though !

If you get a bunch of his pics from when he was younger together and from him growing up, funny pics, naked baby in the tub, his and his "boys". Make a PowerPoint presentation and play it on a TV during the reception and bring out a separate "birthday" cake and have everyone sing him happy birthday!

You are sure during the ceremony and not later when you are at home correct? If you mean during the ceremony you could play his favorite song, or have somebody sing a song that he really likes. That is kind of a hard one.
If you want to wait until later, if you know where you are going the night of your wedding and it is a hotel see if they will bring wine and strawberries up to the room, then buy him a gift that he would want. You could also surprise him on your honeymoon by planning a day of it or something. I hope this helps a little. This one is kind of tricky.
