It is a matter of geography really. A theater of war usually entails a COCOM's (Combatant Command) area of responsibility, like CENTCOM, PACOM, EUCOM, and covers a wide goegraphical spectrum that could cover what we recognize as various parts of mutliple continents, basically, a really large area.
A Theater of Operations is much more localized that could involve one or more countries, regions, or even a continent. During WWII, the Theater of War was Europe and Africa, stretching into SWA. Typically we in the military refer operations now to strategic, operational, and tactical levels of war when talking about hostile engagements.
Even more localized is the definition of a combat zone. Right now, CENTCOM has a number of countries where service members are working towards the war effort in Iraq. If you are in Iraq, Afghanistan, or Kuwait, you are considered in the "Combat Zone". But I can be a member of the CENTCOM staff and be stationed in Quatar or Barhain which is considered the COMMZ (Communications Zone) which isn't eligible for combat pay and other incentives afforded soldiers in the Combat Zone, but are still forward deployed in support of hostile actions. Clear as mud?
Start with FM 3.0 (Operations), and if that doesn't work, then look up joint doctrinal manuals covering doctrine for all forces of the Department of Defense.

Combat "zones" are usually limited to a specific Geographical area like a point within a country or the area of specific sea. A theater of operation is about the size of a Continent or Ocean.
