I would honestly prefer that they not come to the table at all, but if they do, how do you give them the money, and how much should it be? Is it more if you actually request a specific song?

This is like the squeegie guy... you sort of have a duty to nod 'No Thank You'. Failure to do that can be construed as a tacit acceptance of his services. Assuming you don't mind this service, then paying them right then and there in cash is fine. If you request a specific song it'd probably be expected that you tip a bit more because theoretically you've enjoyed the service even more...but since tipping is at the discretion of the tipgiver, you have wide lattitude. The price of the restaurant, whether singing is an advertised or known feature of the restaurant, and what the custom at the restaurant is also all play a role in determining the proper tip amount.

I always have paper and pen with me or if not ask someone at the front desk. I slip a nice sentence like " You have a beautiful voice thank you for making the night more memorable" and tuck a few $ inside and ask that it be give to him or her from the manager.

Imnotbtami, don't ever go to the same restaurant twice, or you will get some "extras" with your meal.
