In order for the second sentence to be correct, you need to make "ones" singular to read:
One takes this item.

Technically, that is an incomplete sentence.
I'm assuming you are just asking about a part of a sentence.

For example, if the item was a dictionary and the person speaking, was a teacher, addressing her students, she might say: "The ones who take this item,(dictionary) must return it to my desk, before leaving the classroom."

That would be a complete sentence.

I think it would be the former, but doesn't it depend on context as well? What's the full sentence? "Ones who take this item" is NOT a complete sentence.

First of all, you shouldn't use the word 'one' in a sentence unless you are using it as a number. You could do "he/she who takes this item" or something. At least, my english teacher thought that way and it's contagious I guess.

actually i believe both phrases are wrong.

ones who take this item. bad grammar all together

ones take this item. still bad grammar but better. a plural noun must be follwed by a plural subject. i think the correct sentence would be

one takes this item
