Does your local college have dental students that will do this for free or a nominal charge. I know that Marquette University in Milwaukee does some dental work for their clinicals but to what extent I don;t know.

I went to the county and had it done. I was in college at the time, the health center refered me.
But it was scary as hell, the guy walked up with pliers - I am not kidding. Yanked the dang thing out. Not garage pliers.
