Couple of ways. The stranger could have been in the car before, or the wife could have let him in thinking he would help. Or the stranger was a baby she gave birth to while he was gone.
I am assuming there is more to this riddle than what you have written, because there must be some twist, but there was not enough information to rule out any of my answers.

The original couple consisted of a the man and stranger. The stranger was in the car to begin with. While the wife is dead, you did not say she was originally in the car or that the stranger had anything to do with her death.

I guess the stranger must have climbed in the unlocked or ajar trunk of the sedan and then pulled the trunk lid closed. If the man was blind (had never seen his wife) and came down with a sudden case of amnesia or schizophrenia (his wife was a stranger to him) after returning to his car, it could be his dead wife in the trunk.

Maybe the wife let the person in the car and then the stranger killed her. Or maybe the man has amnesia now and thinks his wife is dead and the "stranger" really is his wife....but then how did he find his way back to the car.....hmmmmmmmm..........
