OSHA has a number of classes, given at one of the OSHA regional training centers or by an outreach trainer. The only test is if someone is trying to become an outreach trainer for the OSHA 10 and 30 hour classes.
Some companies hire a consultant or use an on line service to give the 10 or 30 hour class and include a test to assess knowledge. Technically the test results should not affect whether the person passes or not because once they take the class they are eligible for a certificate, however that's how the company makes money, through retests.
That having been said, I work with safety managers/coordinators all the time. There are many knowledgeable people out there, there are also a lot of people who get the position through seniority or because they can perform their old job anymore. If your company has a good safety program, the person who administers it is of less importance. If the company doesn't have a strong safety program and relies on the knowledge of one person to drive safety the program is doomed to failure, and the incompetence of the person will negatively impact safety.
Plants that rely on "the safety guy" and don't have a safety program that is driven from the top down through all levels of supervisors and employees needs a competent person to keep the program moving forward.
But to answer your question, if the person flunked the test they should be made to pass it, if for no other reason, to give them credibility with the rest of the employees. Some people don't test well, especially older employees so its not uncommon for them to have to retake tests.

Jesi has it correct. There are no required tests to be a safety person, but if they take a course and fail, it should raise red flags.

Only if they haven't retaken the test and passed it. If that is not the case, then it should be reported to your local employment authority. That's terribly dangerous and a HUGE conflict of interest.

I think that the person should have the opprotunity to study and retake the exam. But after that you should consider replacing him/her.
