I think maybe trying to keep baby-daddies involved in these kids lives whenever possible. Then again, most of the baby-daddies are sh*ts.
If the pop culture would not promote and romanticize violence, that would be a great start.

It has to start with a solid family foundation. Then we need responsibility. If someone does anything wrong anymore the first thing people say is it was because of their home life not their fault. B.S. if you do the crime do the time. Take responsibility for your actions and quite blaming someone else or society. You did it, your fault.

Raise kids up in a moral environment instead of in the home of an unwed mother who has a baby every year and sleeps with a dozen men. Make them face adult consequences when they commit a crime no matter how young they are.
