Basic info should be on the outside like what is listed above. BUT on the inside of your luggage you should include a copy of your itinerary as well as all contact info at your origin (home) as well as your destination. Place it inside a ziploc bag and on top of your items. This will help identify your luggage should the tags be ripped off in transit. The agents will go inside the bag to find identifying material to contact you.

The ONLY Information that should be on the tag these days is your NAME, your Telephone number and that's it.
To often, criminals will haunt the baggage claim areas and get the informatoin of where people are headed and compare this with where they live. Easy to go break into a house that you know the folks are not home. The phone number is there ONLY in the event thta you have to either identify the bag, or it is lost and they need to contact you. Business men put thier business card in the tag when travelling. It's only for contact, don't give the bad guys any more than you have to.

Name & phone number (cell phone if you're carrying one), work address, and a throw away e mail address. I use the yellow luggage tags from Magellans.com that you can put your itinerary in in case your luggage gets lost . This could be useful if you're hitting several cities or countries in your travels. Make sure you've got a copy of your itinerary inside your suitcase, too.

Name, address and phone. I also put the same information inside my luggage, in case the luggage tag gets pulled off.

I don't put my first name...just my first initial and last name. I do put my address...but it's also a great idea to put your work address down. I don't put my phone number down...but I do put an e-mail address down, not the one I use most often though.
Inside my luggage, I put down information such as the hotel I'm staying at, their phone number and address and my name.

Phone number very important if you are arriving at your dest. you want you luggage deliverd to that location instead of your home address.
