If you are a British Citizen and have a machine readable passport, you fill out the I-94 form on the flight across the Atlantic. You had it to the immigration officer when you land and they assess the form and application. Generally they give you entry clearence. If you have a criminal record or plan to marry or work (and the such), you have to apply for the visa before you leave the UK. If it is just for a vacation you dont have to apply for a visa before you take off.

i dont think you need a visa to travel to US, normally as you enter you are automatically as a tourist for three months, the officials stick a paper on yer passport and it will be removed the day u leave US. But i cant be 100% sure cuz i am a US passport holder, so try the embassy just in case.

Technically yes, but you can get it on the plane. Little green card they staple into your passport. Your MACHINE-READABLE passport!
