When filing out Jury duty form, Prescott Division - #14, requesting excuse, (5-10) not app. this district. ???

#14 (5-10) not applicable for this district. Can anyone tell me what this means? do I fill in the circles? 1 - 4 do not apply to me and I do not know about the rest nor where to look for an explaination.

If I understand the problem, the form give you the option of stating that you dont' live int he district for which the form is generated. Beore using this, I'd check with the Jury office.
