Get a blender that is strong enough to crush ice cubes cos those little hand held blenders cant do it.
You put the fruit you want to put in the smoothie and which it up. Then add the ice cubes and keep on blending them in until the smoothie is the consistancy you want. You can also add chilled water if you want too. With just ice cubes and water, you should experiment with fruits so that it suits the level of sharpness you want. Some people find that things like kiwis too sharp, for others, just bananas and strawberries are too bland. Each to thier own.

A favourite of mine is:
500ml of orange juice, 1 banana, a handful of strawberries/fruits of the forest, 1 kiwi fruit and a mango. Blend this with 5 or 6 ice cubes and you'll have a refreshing and extremely healthy smoothie.

Here is another recipe for ya. 8 oz of apple juice, 1/2 a banana, 1 cup ice, any 1/2 of each of 2 types of seasonal berries such as raspberries and blueberries, they work awesome together. This is very low calorie and you can also add a protien supplement if you desire as well with out it tasting like cardboard :)

I use frozen berries, water and add crystal light lemonade powder. Adds sweetness.

Sounds potentially bland.
