I was in an accident...other guys fault. How do I locate his insurance company if on the report it says "Block 25:out of state insurance code". He's from PA if that helps any... I have a name address and policy number but no insurance company name or codes.

You are supposed to get that information from the other driver at the scene of the accident.
Call the other driver and ask them the name of their ins co.

Your insurance company probably won't help. Mine didn't. They weren't liable so they left it up to me to fight the other company.

If your insurance policy has a clause that covers you if you get hit by an uninsured driver try to make a claim. (most policies do)Then they will find out who the other guy's insurance company is and tell you who to file a claim against when they reject your claim..

Due to privacy concerns, chances are the motor vehicles department won't give up that information. This is something you should have gotten at the scene. If it's not on the police report, about all you can do is write to the name and address you have, cite the policy number and request more information. I'd send it certified just to prove you requested it. If you don't hear in a reasonable time, and your insurance agent is less than helpful, I'd contact an attorney for advice.

Your DMV should be able to help. When my daughter was in a car accident, she was given wrong insurance information by the person at fault. We had to drive there and they gave us the up to date information right there. If your DMV doesn't have that info, call Pennsylvania Department of Transportation. I am a PA resident and they are real sticklers here about having insurance and carrying proof of insurance.

Try calling (717)787-2317 at PA DMV they may be able to give that info or go back to police station and have them obtain code.
