When you call, you are usually connected to a trained individual or counselor who can talk with you or refer you to an agency or hospital for treatment. Someone who is trained to deal with a suicidal individual will talk with them about whatever problem they are going through in order to help them. Depending on the helpline, sometimes they will dispatch a team to assess and assist with hospitalization if it is needed. It depends on the States laws for psychiatric screening too.

You call them and talk to them. I called once because my crazy ex kept telling me about how he wanted to kill himself and I wanted advice.
If you want to commit suicide, call them. If you need help, call them. You don't have to give your real name, they're there to talk to you and listen.
