Not necessarily. Success can be a good teacher, too, because you may develop skills you never knew you had. As the saying goes, "Success breeds success". People can certainly learn from failure, but it isn't necessary to fail.
Of course, making mistakes is part of any learning process, but it isn't failure as long as you're trying.

Technically, mistakes are a form of failure. You failed to do something right, thus resulting in a mistake. But yes, mistakes are one of the best ways of learning things. Not highly recommended, though.

No, making mistakes is, because you learn not to do it again. In most common people. It's as if your doing something and lets say you got burned, and you just learned not to do it again, from your mistake.

No, sometimes it's unavoidable. When you strive and struggle with all your being, and outer influences cause adverse outcomes then you have no control over the results.
