I assume you're talking about in a hospital psych unit. It would be in the unit policies. In Minnesota, if you are on the unit and you had been put in restraints or seclusion, you could ask to see them and they would have to show them to you if you are 16 y/o or older. If you are out of the hospital and want to sue, your attorney can get them. In the 22 years that I worked on adult or adolescent psych units, I never saw or even heard of anyone being being put in restraints unless it was absolutely necessary.
