Movies or a school dance for the first date so you can avoid awkward pauses in conversations because the movie will be playing or you'll be dancing to music. After a couple dates like that you can go out to eat to a small diner or something so your more comfortable with eachother and theres less of a chance of getting uncomfortable during your conversation.

Depends on how well I knew them.
I generally keep fairly busy and I would just invite them along to something I already do in my everyday life.
If you're trying to break the ice, find out if they interested in something (wine tasting, karting, coffee, art, etc) and plan around that.
I'd try and stay away from movies for a 1st date... you can't talk to them and get to know anything about them.

1st date - keep it short and simple - go for a coffee or a beer (if you are over 21) or an ice cream at the park. Anyplace where you can sit and chat. You don't want to commit to too much time with this person until you are sure that you click with them.
