I think that you need to be open with your partner. Tell her that its something that you would like to try, but let her know that in no way does it mean that your feelings for her have been reduced or that doing so will change your relationship. Trying something new can be fun for both of you, but try to also understand it from her perspective. Would you have a 3 some with another guy if she wanted to? Once you think of what your concerns would be, try to see it from her point of view and what her concerns may be and try to address them. If it does happen, try to balance yourself between both women so that she doesn't feel left out or not pleasuring you the right way. Let her know that the 3 some in no way means that she isn't a good partner, but something you want to try. There is no better way than communication when it comes to new sexual endeavor.

That depends. Is she the "love of your life" or does she think she is? If so, say bye-bye to the fantasy unless she brings it up, then explore the question playfully at first, then get more serious. COMMUNICATION is always key in these situations.
If the relationship is just about a jolly good time and you both know it, just ask. Talk about it and feel her out. If she's grossed out, starts screaming and or crying, give it up. If she's just shy, keep COMMUNICATING!!!!!

If she is attractedto females... then go for it. But be careful because if you ask her andyou are not 100% sure she would do it... you may lose her trust or make her feel like she cant please you. Be careful...
