Google "power bar recipes." You'll find all sorts of recipes for how to make your own homemade power bars. I make them all the time for my husband who is an elite athlete. Most of the recipes start out being made in a baking pan, so you have the beginnings of a sheet cake. Find a recipe that sounds tasty, and then make a healthy "frosting" with something like flavored fat free cream cheese, or the like. Spread over it and then decorate like a cake. Voila, you have a healthy birthday cake. I think that would go over very well with your guest of honor.

Bodybuliders like to eat protein... so Chicken, steak, protein shakes or make a frozen protein shake pie. Better yet, jump out of a pretend cake with dumbbells in hand or have a party at the gym! Find out what the body builder likes. I am sure for one day it wont hurt to eat the sweets just once for a birthday.

I wouldn't hesitate to form a gorgeous, beautifully arranged cornucopia of fresh fruits, nuts, and perhaps some glace apricots (not dried!), if they can be had where you are.

I'd take a look in some cookbooks or online sites for especially festive ways to cut, carve and present the fruits so they are displayed to best advantage.

Try a fruit plate...pineapple, apples, melon, raisins, etc. You may also go for the meat/cheese selection platter (though it's a bit pricey). An ice cream cake may work for a birthday cake. I believe that Dairy Queen makes a no/low sugar variety.
