If there is a custom like that in the company you're working for, maybe you could write a thank you card. If not, I think it will be sufficient to say to your boss something like "thanks, I really appreciate that you evaluate my work on this way, I'll try to work even more in the future because I know I can do it".

DO NOT send a thank-you card. This would make it look as though you're in shock that they gave you the promotion, like you can't believe it happened.
Next time you're talking to your boss, you can thank him or her casually - Thank you so much for giving me the opportunity. I'm having a great time on this project, etc.

Nothing. Getting a raise and promotion is something that you did well, and you should not have to do anything in return except continuing working hard and smiling.
They will appreciate your continuing hard work. Be polite and responsible, and don't let his raise make you lazy.

No, no thank you card or letter is needed! Did you thank them when the promotion was extended to you? That was your opportunity to thank them if you didn't. If you didn't thank them when you accepted the offer, you still can stop by your boss' office and let him/her know how much you appreciate your reward. You can show your gratitude by continuing to keep up the good work! Show them your promotion was well-deserved!

Thank your boss directly without making too big of a deal about it. Then try to live off what you were making before the raise and put the raise into savings.
