Contact your group health insurance company. They will have a form for you to use. Be sure to mail it "certified" mail so that you will have proof of receipt.

COBRA is an option that terminated employees have by law. Procedures and timeframes for informing the employee about COBRA are standardized and must be adhered to by the company and the healthcare provider. The terminated employee has a right to know and understand their options so they can make an informed decision about their healthcare. You need to contact the healthcare provider and sort this out. As I'm sure you know healthcare is on top of the list of importance for a families security and happiness.

I dont have anything on Cobra but I can tell you of a better health care option for your terminated employee and it WILL NOT cost you a thing. Go to www.mybenefitsplus.com/40396616. It is definitly at least worth checking out. I was given the option to have Cobra many years ago when working at a Temporary agency and it was not helpful to me or my family. I don't know if things have changed since then but these benefits are what My husband and I have for us and our four children. It is an Awesome Benefit Plan!Email me if you have any questions. Google "federal cobra" and look for the federal web site ending in ".gov" Go to it and use the search function to find "cobra letters". The site has sameple letters you can send.
