It is very quick and easy to do it through the State Dept. website:
http://travel.state.gov/passport/passport_1738.html
They provide the forms, instructions, lists of offices, information on expedited service, etc.
It's a very easy process. Good luck.

Depending on where you live you may be able to apply through the post office alternatively the department of state has offices in major cities. Got online to the Department of State - and click on passport requirements. If you are more than six weeks away from travel - otherwise if you need to travel very soon you can pay a fee to get it expedited.

get 2 passport photo's taken...a copy of your original (must have raised seal) and another form of Id..check with you local post office...They can usually do it...it will be $65 dollars for the passport and $35 dollars for the post office processing fee. You may also get the forms from the post office and mail them into the nearest passport office for processing or print forms from on line and mail to nearest passport office.

For US passports, apply at County Courthouse or some Post offices.
You need photos, birth certificate, another for of ID and completed forms.
