1. How are you: Como estas?
2. How old are you?: Cuantos anos tienes?
3. What grade are you in?: En que grado estas?
4. Where are you from?: De donde eres?
For the sites, search yahoo or google.
Oh, and these translations are in the familiar form so you use them if you are talking to a person who you know (like a friend or family member) or who is younger than you.

1: Como estas?,...2: Que edad (tu) tienes?...3: En que curso estas?... 4:De donde eres?..4a De donde (tu) eres?..4b De donde eres (tu). some words can be added, it all depend to whom you are making this questions, some times you need to be more formal especially if questioning an older person, as well that there is a different went such questions are directed to a man a/o a woman. Questions can also have a layer of innocence, direct or indirectly some time you will ask because you have no other choice, such as to brake the Ice?, (tu), you can accentuate the question by using (tu)= (you) in the end, such as "Que edad tienes tu?, also older people will show more respect to younger peoples by using almost the same format. Remember that Spanish is more expressive and sometimes a little more physical (body language),..(body expressions), and not just by moving hands, like Italians? In my oppinion? to My knowledge?

Just DO NOT use the words of "teddy bear" but the ones of "JAT".
Teddy bear is translating literally word by word, this changes completely the meaning. Probably the intentions are good, but unfortunately mistaken. (sorry my english, but I am native spanish speaker)
so do not get confuse... and good luck.

I'm going to assume you're speaking to a peer.
1. ¿Cómo estás? (You could throw in a "tu" at the end, especially if your saying it as a response.
2. ¿Que edad tienes? or ¿Cual es tu edad?
3. ¿En que grado estás?
4. ¿De adonde eres? or ¿De adonde provienes? or ¿Adonde nacistes?
