Any printing company will do this for you as long as you can send the design in a format that they have, eg JPEG, TIFF etc.
My Fiancee works for a printing company so I know. They can cut out hearts and other shapes usually, or cut slits in if you wanted to put ribbon through it. just phone a load up for quote and see who is cheapest. Cheap doesn't necessarily mean poor quality either, they may just not be as established or have a small workforce so can drop their costs.

Use some design software (MS Publisher, maybe) and create your own... Then use Adobe Online to make them into PDF's and take those along to a printers and see if they will do them for you...
There are a number of companies around the country... Just check out online (www.yell.com - if you are in the UK) or in the phone book!!!

The quality is really good too.
