The Family Edition of the Amazing Race was last Amazing Race 8. I'm not sure when the next Family Edition will be. Amazing Race 9 is also over. But for information, and maybe updates on when the next applications will be, visit http://www.cbs.com/primetime/amazing_race_application/

Go to www.cbs.com and look up the "Amazing Race". They will have instructions. However, I have not heard that they will be having another family edition.
