I would assume that you'd have to be part vampire to have one, as (I hear) everything you eat for the first month or so tastes like blood because you keep hitting it with food. Personally, I prefer the taste of food to blood, so I will refrain from piecing my tongue.

In most states, body piercings of any kind require you to be 18 or to have parental consent.

You need to be 18 to do it without parent permission, but if you get a permission slip from your parents you can get it done.
