If you need the actual types of goals to write, you can look at the Desired Results template at www.cde.ca.gov/sp/cd/ci/drdpforms.asp
We use this form in California to assess our children. It is easy to use, has actual examples of what you are looking for, and has several different templates for varying ages from birth to 12 years. As your children advance through the stages, you simply move up to the next template and start assessing them for that one's skills.
A very comprehensive assessment is the HELP Strands... (Hawaii Early Learning Profile). This is used to write goals and assess children with delays. It has lots of very specific, small, incremental steps that children naturally progress through so that you can see what skills come next. You can find and order the HELP Strands at www.vort.com/products.
