How do you stay focused all the time? Hmm, I'm not sure that is entirely possible, and I'm unsure of what you want to be focused on. However, in the case of my attention waning from my college readings, I was interested to learn in my freshmen psych class that simply telling yourself to "concentrate" or "think" actually works! Or at least it did for me.
Other suggestions are eating and exercising right. A healthy body really does accentuate the mind. There are chemical reasons why people with excess body fat have a higher tendency to be lethargic.

Some students are more intense than others about studying. Some have ADD (attention deficit disorder) or some similar difficulty that makes it hard to stay focused. But normal people don't stay focused ALL the time.
My daughter has ADD and has to get up from her studies every so often and just walk or dance around the room or house to release the build up of energy. I have the same problem. Too much energy.
Her roommates at Yale University would kid her about it. But she was very persistent and graduated cu m laude from Yale with additional high honors in her field of biological sciences.
She starts graduate school this fall on the west coast at a different university and I have no doubt that she will have to deal with the same issue all over again.
Forgive yourself for your limitations and find the work arounds that do it for you. And don't ever let your limitations limit you. We all have limitations and we all have to find the ways around them.
Make friends with the students who are the super achievers in your school and mimic what they do and see if it works for you.

Stay in the moment. It takes practice. Meditation helps as it forces one to be more focused.

The thing is you can't the key is to try to stay focus most of the time. Give your self a break. I read your profile and your quite a busy bee. Just give your self some me time and relax and I'm sure everything will workout.

practice practice practice... stay focused for a few minutes.. take a break.. do it over again...increasing the amount of time you stay focused... you have to build up your ability to concentrate for long periods of time.
